Legend has various stories about how this form of Lord Shiva originated. Lord Shiva along with Goddess Parvati decided to transform themselves into an antelope / deer. They roamed around in the forests of Nepal along the Bhagmati River. Very soon the Devatas began their search for them. When they traced Mahadev and his divine consort they requested them to return to their original forms. Since the Lord himself had changed into an animal and coexisted with other living beings he was given the name of Pashupatinath.
